The purpose of this survey is to gather data on how many daily OTs and LTOs are losing prep periods for the school year 2024-2025.

*Please prepare a written document that includes job number(s), the dates/time, prep loss subjects, school name(s), Principal Name and indicate if Daily or LTO position, and gather any other important information which will be collected at a future date. (i.e. Timetable of the teacher you are replacing). A template for this information can be found here:
