Reporting

Below are the three TDSB Health and Safety Reporting/Forms that TDSB elementary OTs may have to refer to for documentation/reporting. These forms are submitted to the TDSB Health and Safety office.

We have also included a link to the Safe and Caring Schools Incident Form (Part I and II) which is not dealt with by the Health and Safety Office, but submitted directly to the principal.

ERA (Employee Report of Accident of Injury)
PAPER COPIES ONLY

This form is completed by a worker who suffers an injury.  If the worker is unable to complete the form, the Supervisor may do so on their behalf.

Employee’s Report of a Workplace Violent Incident (ERWVI) 
ONLINE ONLY

This document is completed by a worker if an act of violence has occurred. An Act of Violence is defined as “threatened, attempted or actual conduct of a person that causes or is likely to cause physical injury.

Click on the Employee Report of Workplace Violent Incident (ERWVI) link and fill out the online form.

Concern/Near Miss Incident Form

This form triggers an investigation by the Principal and the Principal must respond to the Occasional Teacher within 5 business days of receiving the form. Teachers should include details like what it made you feel like, threatened, harassed, scared, whatever it felt like for you and also any behaviours this student engaged in with you that were challenging.

This document is completed by a worker if they have a concern. A concern is defined as “a potential or existing hazard which presents risk to the health or safety of individuals in the workplace.”

Safe and Caring Schools Incident Reporting Form
PAPER COPIES ONLY

The occasional teacher completes Part 1 of the form and submits it to the school principal. The Occasional Teacher will fill out part 1 and give it to the Principal along with Part 2. The principal then completes Part 2 of the form and provides a copy to the OT submitting Part 1.
